The Southwest Regional Public Health Training Center was established in Fall 2010 to enhance the competency of the public health workforce in California, Utah, and Nevada.
This Privacy Policy (the “Policy”) applies to the access and use of the srphtc.ucla.edu website, which is a private online community, and any successor sites there of srphtc.ucla.edu”. srphtc.ucla.edu ("srphtc.ucla.edu," "we," "us"), provides srphtc.ucla.edu and all site-related services (collectively, the "Site" or "srphtc.ucla.edu") to authorized users ("users").
This Policy describes our practices in connection with PII that we collect through our website located at www.srphtc.ucla.edu (the "Site"). This Policy does not address our practices regarding information that we collect through any website, or by any other means, other than through the Site, nor does this Policy govern the collection, use or disclosure of information by third parties.
By using the Site, you agree to the terms and conditions of this Policy. If you do not agree to the terms and conditions of this Policy, please do not use the Site. We reserve the right to change this Policy without prior notice. You can determine when this Policy was last revised by referring to the "LAST UPDATED" legend at the top of this page. Any changes to our Policy will become effective upon our posting of the revised Policy on the Site. Use of the Site following such changes constitutes your acceptance of the revised Policy then in effect.
Note Regarding the Use of the Site by Children: The Site is not directed to individuals under the age of thirteen (13), and we request that such individuals do not provide personally identifiable information (“PII”)
THE INFORMATION SRPHTC.UCLA.EDU COLLECTS
Personally Identifiable Information
srphtc.ucla.edu collects PII about its users (and, in certain cases, a user’s parent or guardian) at the time such users register to use the Site, including (a) the name, address, phone numbers and email address of the user, and (b) if the user is under eighteen (18) years of age, the name, address, phone numbers and email address of the user's parent or guardian. At the end of the registration process, the user is given a screen name (which for some users also functions as an email account) and password. To enable users to participate in some of our online activities and features, srphtc.ucla.edu may ask them to send srphtc.ucla.edu an email message or fill out an online form that may include PII. We seek to ensure that any such PII collected by us, including the email address provided by the user, will be used by srphtc.ucla.edu for purposes relating only to such online activities and features. Also, users may submit, upload, reveal or otherwise provide PII through their use of the Site, participation in online activities (including through the provision of videos and photos), games and contests, or in chat rooms, blogs and discussion areas.
Non-Personally Identifiable Information
srphtc.ucla.edu collects some information about its users which is not personally identifiable to such users (“Non-PII”) and which helps us improve the design and content of the Site and make the Site more interesting and useful to users. Non-PII includes aggregated information, demographic information and any other information that does not reveal your specific identity. We and our third-party service providers may collect Non-PII from you, including your Internet Protocol (IP) address, MAC address, your computer type, screen resolution, OS version, Internet browser, as well as information collected through cookies, pixel tags, web beacons, and other similar technologies, to better serve you with more tailored information and facilitate your ongoing use of our Site. We and our third party service providers may also collect Non-PII such as demographic data, for example your screen name, your location, and/or your disease, as well as PII that has been aggregated in a manner such that the end-product does not personally identify you or any other user of the Site, for example, by using PII to calculate the number and frequency of users and visits to particular areas on the Site and the number and frequency of users who participate in an activity, contest or game. HOW SRPHTC.UCLA.EDU USES THE INFORMATION IT COLLECTS
Personally Identifiable Information
srphtc.ucla.edu uses users' PII in the following ways:
(1) to fulfill requests, such as to enable users to subscribe to our online and email newsletters (if you would prefer not to receive such newsletters, please follow the opt-out instructions at the bottom of any such newsletter that you receive from us);
(2) for administrative purposes, including to send users important information about the Site (because this information may be important to the use of the Site, users may not opt-out of receiving such communications);
(3) to inform users of programs and services that may be of interest to users (if you would prefer not to receive electronic marketing messages from us, please follow the opt-out instructions at the bottom of any such message that you receive from us);
(4) for our internal business purposes (e.g., data analysis, research reports, audits, enhancing the Site, improving Site services and identifying usage trends);
(5) to permit users to use the functionality of the Site to, for example, express opinions on various topics, provide feedback and comments about features of the Site and send instant messages;
(6) to introduce users to new features, areas or content of the Site; and
(7) to permit users to participate in games, sweepstakes, contests and similar promotions (collectively, "Promotions"). You should carefully review the rules, if any, of each Promotion in which you participate, as they may contain additional important information about our use of your PII. To the extent that the terms and conditions of such rules concerning the treatment of your PII conflict with this Policy, the terms and conditions of such rules shall control.
If a user is under eighteen (18), we may use the PII of such user’s parent or guardian to obtain parental consent for such user’s use of the Site.
Non-Personally Identifiable Information
Because Non-PII does not personally identify you, we may use such information for any purpose. In addition, we reserve the right to share such Non PII, which does not personally identify you, with third parties, for any purpose. In some instances, we may combine Non-PII with PII (such as combining your name with your geographical location). If we do combine any Non-PII with PII, the combined information will be treated by us as PII hereunder as long as it is so combined.
HOW PII IS DISCLOSED
We may disclose your PII to our third party service providers who provide services such as website hosting, data analysis, infrastructure provision, IT services, email delivery services and other similar services to enable them to provide services.
We may make available through the Site services (for example, message boards, chat rooms, profile pages, blogs, activities, games and other communication forums) (each, a “Forum”) to which you are able to post information and materials. Please note that any information, text and images posted or disclosed by the user on or through such Forums becomes public information, and may be available to visitors to the Site and to the general public. In addition, certain PII, such as your name, may be available for other users to view when you make a posting to such Forums. Information regarding your activities in such services may also be available for view by other users (for example, other users may be able to view a list of all postings you have made in all available Forums). We urge you to exercise discretion and caution when deciding to disclose your PII, or any other information, through a Forum or otherwise through the Site. WE ARE NOT RESPONSIBLE FOR THE USE OF ANY PII YOU VOLUNTARILY DISCLOSE THROUGH A FORUM OR OTHERWISE THROUGH THE SITE.
We may provide functionality to permit you to send messages to other Site users through the Site. Please note that if you use such functionality to send such a message to another Site user, your screen name will be disclosed to that user as well. Further, if a message you send using such functionality contains PII, we cannot control how the PII will be used or disclosed by the recipient of your message. We urge you to exercise discretion and caution when deciding to disclose your PII, or any other information, in any message, and to be careful about the people to whom you send such messages. SRPHTC.UCLA.EDU IS NOT RESPONSIBLE FOR THE USE BY OTHERS OF THE INFORMATION THAT YOU DISCLOSE IN SUCH MESSAGES.
We reserve the right to share the PII you provide in connection with Promotions, with third parties sponsors of such Promotions (irrespective of whether such Promotions are hosted by us) or otherwise in accordance with the rules applicable to such Promotion. You should carefully review the rules, if any, of each Promotion in which you participate through the Site, as they may contain additional important information about a sponsor's or other third parties’ use of your PII. To the extent that the terms and conditions of such rules concerning the treatment of your PII conflict with this Policy, the terms and conditions of such rules shall control. Further, in some instances the screen name (combined with age and gender) of a user, along with a user’s submission to a Promotion, may be posted on or transmitted through the Site in connection with a Promotion. For example, if a user wins a picture drawing contest for a picture submitted to the Site, the picture and the user's screen name, age, and gender may be posted publicly on the Site. Please note that certain Promotions and other features of the Site may be hosted by one or more third parties, and such third party’s user agreement and privacy policy, not this Policy, may govern your use of such Promotions or features.
We may link from the Site to a third party payment service to permit you to make a donation or contribution. If we do so, and if you wish to make a donation or contribution through the Site, you will be directed to a web page hosted by the third party payment service (or its service providers) and not by us (such page, the “Third Party Page”). Any PII that you provide through the Third Party Page will be collected by the third party payment service and not by us, and will be subject to the third party payment service’s privacy policy, rather than this Policy. We have no control over, and shall not be responsible for, the third party payment service’s use of information collected through the Third Party Page. PLEASE NOTE THAT THIS POLICY DOES NOT ADDRESS THE COLLECTION AND USE OF INFORMATION THROUGH THE THIRD PARTY PAGE OR THE PRIVACY OR INFORMATION PRACTICES OF THE THIRD PARTY PAYMENT SERVICE.
We may occasionally post information on the Site regarding one or more research studies or one or more researchers who are seeking information. Please note that any PII you provide to any such researcher or in connection with any such research study will be provided to the applicable researcher and/or the individuals and entities conducting such research study, and not to us. We urge you to exercise discretion and caution when deciding to disclose your PII, or any other information, to a researcher, to an individual or entity conducting a research study, or to any third party. WE ARE NOT RESPONSIBLE FOR THE USE OF ANY PII YOU VOLUNTARILY DISCLOSE TO ANY RESEARCHER OR IN CONNECTION WITH ANY RESEARCH STUDY, WHETHER INFORMATION REGARDING SUCH RESEARCHER OR RESEARCH STUDY WAS POSTED TO THE SITE OR OTHERWISE.
We reserve the right to transfer any and all information that we collect from Site users to a third party in the event of any reorganization, merger, sale, joint venture, assignment, transfer or other disposition of all or any portion of srphtc.ucla.edu’s business or assets (including without limitation in connection with any bankruptcy or similar proceedings).
Further, notwithstanding any other provision of this Policy to the contrary, we reserve the right to disclose PII about you to others as we believe to be appropriate (a) under applicable law; (b) to comply with legal process; (c) to respond to governmental requests; (d) to enforce our Terms of Use; (e) to protect our operations; (f) to protect the rights, privacy, safety or property of srphtc.ucla.edu you or others; and (g) to permit us to pursue available remedies or limit the damages that we may sustain. For example, we may report suspected child abuse to appropriate authorities or expressions of suicidal thoughts to a parent and/or guardian.
LINKS TO OTHER INTERNET SITES
srphtc.ucla.edu may contain direct links to other Internet sites. However, please be aware that srphtc.ucla.edu does not control and is not responsible for the content, privacy practices and other terms and conditions governing the use of such other Internet sites or any other Internet sites which are linked to such sites. If you provide any PII through any such third party website, your transaction will occur on such third party's website (not the Site) and the PII you provide will be collected by, and controlled by the privacy policy of, that third party. srphtc.ucla.edu encourages users, parents, and guardians to become informed of the privacy practices and other terms and conditions of all such other Internet sites. This Policy applies solely to information collected by srphtc.ucla.edu in relation to the use of the Site.
SECURITY
We seek to have security measures and tools in place to help protect against the loss, misuse and alteration of the information under our control. Unfortunately, no data transmission over the Internet or data storage system can be guaranteed to be 100% secure. As a result, although we strive to protect PII about you, we cannot ensure or warrant the security of any information you transmit to us through or in connection with the Site or that is stored by us. You acknowledge and agree that any information you transmit through the Site or upload for storage in connection with the Site is so transmitted or stored at your own risk. If you have reason to believe that your interaction with us is no longer secure (for example, if you feel that the security of any account you might have with us has been compromised), you must immediately notify us of the problem by contacting us in accordance with the "Contacting Us" section below (note that if you choose to notify us via physical mail, this will delay the time it takes for us to respond to the problem).
CHANGING AND DELETING INFORMATION
To change or delete your (or, if you are the parent of a user, your child’s) PII collected by srphtc.ucla.edu, and for questions on or further assistance with srphtc.ucla.edu's privacy practices, please use the Report an Issue link. We will endeavor to respond to your request as soon as reasonably practicable.
Please note that we may need to retain certain information for recordkeeping purposes, and there may also be residual information that will remain within our databases and other records, which, irrespective of any efforts by us to delete information, will not be removed therefrom. We also reserve the right, from time to time, to re-contact former users of the Site. Finally, we are not responsible for removing information from the databases of third parties with whom we have already shared PII about you.
